THERE IS A 48 HOUR PRESCRIPTION ORDER POLICY WITHOUT EXCEPTION – SEE LINK ZERO TOLERANCE POLICY TOWARDS ALL STAFF – NO ABUSE WILL BE ACCEPTED UNREASONABLE DEMANDS ON SPECIFIC CLINICIANS WILL NOT BE ACCEPTED AS PER PRACTICE POLICY.

Register with us as a New Patient

Practice Area

Ask the receptionist for a new patient appointment if you wish to join the practice. We usually only register patients who live within a 12 mile radius. (unless exceptional circumstances and it will be at a discretion of a GP).

This practice does not discriminate on age, race, gender, disabilities etc.

Everybody is Very Welcome.

How to Register

If you live in our practice area and would like to register with us, please download and complete the appropriate registration form below and forward it to the Surgery with the documents requested. Alternatively you can contact reception on 028 2563 8800 who can provide you with the appropriate forms. 

We cannot accept your registration until these forms are completed correctly and appropriate identification has been provided. As soon as these are completed you will be accepted onto our list – please note,  you will be registering with the practice rather than an individual GP. 

We cannot provide medication until you are properly registered – please contact your current GP for any medication needed before this is completed. 

Please be advised when signing documents to become an Organ Donor patients must be 18 or over. Parents or guardians signing on behalf of under 18’s is not acceptable.  

When presenting to register you will need the following:

  • HS200 for NORTHERN IRELAND ONLY. This form must be completed if you wish to register with our practice. You will also be required to supply Photographic ID & Proof of a current Northern Ireland address. (Please refer to our registration policy below).                             
  • HSC/R1 –  This form is used for individuals entering or returning to Northern Ireland . You will also be required to supply Photographic ID & Proof of a current Northern Ireland address along with other documentation listed on the form. Any person registering from outside the UK/ROI must submit a copy of their Visa or Work Permit.

  • **Please be aware of our Practice Prescribing Policy as this will be applied to all new registrations**

If your application is refused

If you live over a certain distance away from a practice, or cannot provide the correct documentation, we may refuse your application. If this happens, you should contact the Business Services Organisation for advice on how to register with a GP.

Registering your baby

You need to fill in a GRO 4 Birth Registration Form. The General Registry Office will give you this when you register your baby’s birth. Bring this form to the GP’s surgery to register your baby as a patient.
